Single Vision Prescription

If your prescription resembles the following, choose the single vision type and fill in the appropriate information as illustrated in the example below.

Bifocal Prescription

If your prescription is similar to the one below, fill in the information as shown in the example.

Progressive Prescription

If your prescription is similar to the one below, fill in the information as shown in the example.

Reading Only Prescription

If your prescription resembles the following examples, please provide the requested information in the format shown below. (Please not you must add the SPH power with the ADD power)
-1.50 + 2.25 = +0.75

-1.25 + 2.25 = +1.00

Reading Only rx

+0.75 - 50 × 10

+1.00 sph

Additional Information for PrescriptionCompletion

OD (oculus dexter) denotes the right eye, while OS (oculus sinister) denotes the left eye.

If "PL" or "DS" appears in your prescription, it indicates a value of 0.

PL (Plano) signifies the absence of spherical correction for the specified eye.

DS (diopter sphere) indicates the absence of astigmatism correction for the specified eye.
